Your Personal Privacy is Important

OSF HealthCare respects the privacy of our patients. Personal privacy is very important to us. We work very hard to make sure the health information of our patients is properly protected.

It is the responsibility of every OSF employee to protect the privacy of our patients’ personal information. Our employees must limit their access to patient information to only the minimum amount necessary to do their jobs. Employees have a duty to report suspected privacy violations to their supervisor or to the Ministry Compliance Team.

The OSF Notice of Privacy Practices describes how patient information is used and disclosed. It also informs patients of their privacy rights.

OSF HealthCare complies with all applicable provisions of the Health Insurance Portability and Accountability Act (HIPAA) and the Health Information Technology for Economic and Clinical Health Act (HITECH)

The health care providers participating in OSF are required to seek your written acknowledgment that you have received the Notice of Privacy Practices.

By furnishing written acknowledgment of receipt, you do NOT indicate your agreement or consent to the uses and disclosures of information described in the Notice.

The acknowledgment indicates only that you have received the Notice. You may decline to furnish written acknowledgment of receipt. In this event, your refusal will be documented.
