Application Process

Please Download and Submit the Following Items

The applicant must perform the following tasks prior to the deadline of May 31 for the following school year: 

1. Download and complete the application form.

   Please Note: You will need to save completed PDF form to your device. Please upload the completed PDF form using the Apply Online form below. 

2. Download and save three reference forms. Fill out the “To be completed by applicant” section and forward to your three references.

   Please Note: Three separate references are required with your application. Two of the three must be from college science instructors. Ideally these are professors who have worked with the student in a laboratory science environment: 

   1. One reference from a biology professor
   2. One reference from a chemistry professor
   3. Additional reference(s) required from the applicant's college advisor or work supervisor, if currently employed in a lab.

3. Submit official transcripts from all colleges and universities attended, including junior or community colleges. 

   Please Note: All transcripts must be sent directly from the school to the OSF Saint Francis HT program; student-issued copies will not be accepted.

4. Submit $50 application processing fee payment, payable online when you apply.

Applicant Selection Process

The program will invite qualified applicants to schedule an interview only after the application and all transcripts have been received. Applicants are encouraged to submit these as early as possible in order to have a choice of interview dates.
